Two copies of this forms shall be submitted with your transcript 30 days before your thesis defense with the required attachment as the follows.
1. Transcript covering the previous semesters;
2. Academic Ethic Certificate.
And the informaiton of your Name, Student ID, Department, Thesis Topic, Thesis Advisor, Committee memebers' information shall be typed before its printing.
*Please note, for change of the oral defense date, the application form for its change shall be submitted 30 days in advance too.
